FEATURED CONNIE SERVICES

The Connie Portal is a standalone, self-service tool that provides authorized users with secure access to patient records and other non-patient-specific applications. The Portal functions as a clinical query, allowing users access to all of the Connie tools, including patient clinical information, a 'snapshot' summary view of key patient data, Connie alerts on patients' healthcare encounters in real-time, a searchable directory of all providers in Connecticut, direct access to the Connecticut Prescription Monitoring and Reporting System (CPMRS), the Referral tool, and the patient consent tool.

Image Share allows authorized users access to patient images in full diagnostic quality. Patient images and reports are available in real-time, improving the speed and quality of care for all connected organizations, from private providers to large hospitals.

Snapshot is a summary of key patient data, including demographics, medical encounters, care team information, and next of kin.

Medication Management is a medication history created using a systematic process of gathering medication information from multiple sources, deduplicating and standardizing the data, and displaying it in a single resource to assist healthcare providers with patient care. With Medication Management, we’ve done the legwork for you by synthesizing the medication information found in your patient’s continuity of care documents (CCDs) and centralizing the information in one place, saving time and making comparative viewing much easier.

The InContext App, also known as the Clinical Information app when accessed through the portal, allows care teams to conveniently access patient clinical information within their own EMR. With seamless authentication, users can retrieve comprehensive records without separate logins. InContext serves as a clinical query tool, enabling access to records from other participating organizations, including prior hospitalizations, encounters, labs, radiology results, and images. This platform enhances care coordination for healthcare providers and patients.

Provider Directory is accessible through the Connie portal and provides a searchable directory of listings derived from multiple public and private data sources. In addition to making it easier for staff to quickly find and contact providers, Connie's Provider Directory establishes a comprehensive, centralized resource for provider information.

Encounter Notification Services (ENS) allows you to get alerts when your patients are admitted or discharged from hospitals and nursing homes allowing you to provide the next right step in your patient’s care.

Connie's AHCD Service centralizes and displays advance health care directives stored in disparate systems, including those registered with ADVault's MyDirectives service, within the patient's clinical information section.
