SERVICES DESIGNED FOR YOU

Connie's goal is to effectively empower patients to advocate for their own healthcare and health outcomes. Connie achieves this by enabling patients to:

  • Exercise greater control over the information being shared.

  • Access shared information between their providers in a centralized resource more easily.

  • Keep track of which of their providers have accessed their health information through Connie.

Our services are designed to be accessed only by individuals who adhere to Connie's data release policy and have signed our data-sharing agreement. This policy was created to ensure compliance with state and federal health information and interoperability regulations, which collectively dictate the circumstances and legal authority under which we can disclose data.

CONNIE PATIENT SERVICES INCLUDE:

Patient Access

Connie is working hard to improve your access to your own health information, regardless of where you’ve been for healthcare in Connecticut. The "patient access" approach enables you to access your health information from your different providers while giving you a choice in personal health applications (PHA) to choose from once confirming your identity.

Patient Portal (coming soon)

The Connie Patient portal will provide you with timely, secure, and direct access to your electronic health information, free of charge. We are designing it to support a single access point for your health information no matter where you are getting healthcare from within Connecticut. The tool will provide a list of your healthcare visits, lab results, medications, noted problems, immunizations, and allergies collected from any of your healthcare providers that have completed their connection to Connie.

Patient Authorized Disclosures

Patients can authorize access to their health records for certain purposes. For example, you can authorize a life insurance company to access your health record for underwriting through Connie. Without your authorization, the life insurance company may not access your health record. Patient authorized disclosures are requested by the patient through the entity the patient designates to access their health record.

Accounting of Disclosures​

State and federal laws give you specific rights about your health data and whether it is shared through Connie. You have the right to request an Accounting of Disclosures, which is a list of healthcare providers who have accessed your medical records. Connie’s technology vendor, CRISP Shared Services, will process your request and issue your report by mail or email, depending on your selection. Opt Out/In

You decide if you do not want Connie to share your health information, you can opt out of Connie at any time by completing the online application. Once you opt out, your health information will be deleted from Connie within 5 business days. Once your data is deleted, your provider will not be able to use Connie to access your health information. However, your providers may continue sharing your health information between each other directly, as allowed under state and federal law, to better support your healthcare needs. Patients who want to opt back in can do so at any time by calling Connie Customer Support at 1-866-987-5514. Unfortunately, we cannot retrieve the information we deleted when you opted out, so your historical clinical information will not be in Connie.​
