Medication Management compiles medication data from various sources, such as Continuity of Care Documents (CCDs), to support medication reconciliation and deprescribing, collaborative care, and clinical outcomes improvement. 

With the help of Medication Management, healthcare teams can enhance the accuracy and completeness of a patient's medication history, leading to more informed and effective decision-making. By reducing medication errors and costs, Medication Management plays a vital role in improving patient outcomes and enhancing the quality of care provided by healthcare organizations.

Ms. Johnson, LPN | Skilled Nursing Facility  

Nurse Johnson has an elderly patient with a chronic health condition that requires her to visit multiple pharmacies and specialists. The patient suffers from memory loss, so tracking her medications has been increasingly challenging. Nurse Johnson relies on Connie's Medication Management to review what has been prescribed for her patient. This best practice has proven incredibly helpful for Nurse Johnson in accurately tracking and managing her patients' medications.

 

Dr. Miller | Emergency Department  

When Dr. Miller sees a patient in the ER who is unable to confirm their current medications either due to their injury or just not able to remember anything other than "a blue pill for my heart", he can look at Connie's Medication Management to quickly clarify this information. On the front page of Medication Management, Dr. Miller can view a summary of all recorded medications prescribed in the last 90 days, including dosages and supply. If Dr. Miller needs more information about why the medication was prescribed or who prescribed it, he can click into the record for complete details.
