How is My Medical Data Kept Safe?

Keeping patient medical data safe is a Connipriority.

Connie  follows all federal and state laws and will only share your medical data for the following purposes: health care treatment, care coordination, quality improvement, and public health. 

Any providers using Connie must follow state and federal laws and agree to keep your health data completely confidential. The Health Insurance Portability and Accountability Act (HIPAA) is the main law that governs health data sharing.  

Connie  uses advanced security measures to keep your healthcare data private and secure. These measures include consistent system checks, state of the art monitoring tools, and next-generation audit capabilities. Additionally, Connie relies on an advisory committee composed of industry experts, representatives from participating organizations, and others who help to inform our privacy and security program. 

What Are My Patient Rights?

State and federal laws give you specific rights about your health data and whether it is shared through Connie. 

You have the right to: 

  • Request an Accounting of Disclosures, which is a list of healthcare providers who have accessed your medical records. Connie’s technology vendor, CRISP Shared Services (CSS), will process your request and issue your report by mail or email, depending on your selection. You can submit your request via this online form or can email, fax, or mail this downloadable form.
  • Opt-out of Connie.  To learn more about opting out please visit our Opt Out page. To opt out, use this online form, or  download the PDF which contains email, fax, and mail options for submission.
  • In accordance with HIPAA, be notified if your information has been viewed by an unauthorized person.
