SERVICE OVERVIEW
We are always expanding our selection of useful tools to help on-boarded organizations get the most out of Connie. One of our standout tools is the Connie Portal, a standalone, secure web-based access point to all of Connie’s tools for our participating organizations. The Portal provides streamlined access with secure login using two-factor verification. The Connie Dashboard, located on the home page, allows credentialed users to launch applications and conduct patient searches quickly and efficiently for all Connie tools in one location.
Core services provide a secure platform for authorized participating organizations to access patient information available through Connie. Access to our services is granted only upon adherence to Connie’s data release policy, and participants must have signed our data sharing agreement. This policy was developed to ensure compliance with state and federal health information and interoperability rules. These rules collectively govern how and under what legal authority we are permitted to disclose data.

Connie Alerts - Our encounter notification services, also known as Connie Alerts, provide alerts when your patients are admitted or discharged from hospitals and nursing homes, giving you the ability to determine the next right step in your patient’s care.

A "Snapshot" summary of key patient data, including demographics, medical encounters, care team information, and next of kin.

Clinical Information includes patient-specific point-of-care clinical tools:

Clinical Data includes overdose alerts, inpatient and outpatient encounters, labs and radiology reports, summaries of care, and separate consolidated lists of the patient’s immunizations and problems. Clinical data also includes a signature service: Image Share, which allows authorized users access to view, compare, download, and import diagnostic quality images seamlessly in near real-time.

Medication Management synthesizes the medication information found in a patient’s continuity of care documents (CCDs) and centralizes it in one place, saving time and making comparative viewing much easier.

Care Coordination includes the patient’s care team, a history of referrals made for the patient through Connie, and any Advance Directives available from MyDirectives.

Social Needs Data highlights when a patient has reported to a provider that they experience problems related to social issues known to impact health using z-codes.

Single Sign-On to the Connecticut Prescription Monitoring and Reporting System’s (CPMRS) Narx report using the Prescription Drug Program (PMP).

A Consent tool with two features:

The Prevention of Harm form, enabling providers to submit written notice to Connie that the patient’s electronic health information (EHI) should not be shared with that patient because it is reasonably likely to cause substantial harm to or endanger the life or physical safety of the patient or another person.

A provider-mediated Patient Consent Tool to support care coordination between substance use disorder (SUD) treatment and medical providers for patients enrolled in SUD treatment programs.

An eReferral Tool to support care coordination with home-based care and social needs services. This tool provides a list of referrals providers have made through Connie.

Provider Directory is accessible through the Connie Portal and provides a searchable directory of listings derived from multiple public and private data sources. This makes it easier for staff to quickly find and contact providers while establishing a comprehensive, centralized resource for provider information.

USER STORIES
John Smith, Medical Assistant | Primary Care
John is the medical assistant for a busy primary care provider. He logs in to Connie every day to check for information on patients who are coming in for their appointments. Through Connie, John can access patient information from hospital visits or visits with other providers, as well as lab and radiology results that he may not have in his EMR.
Jesse Carins, MD | Specialist
Before Jesse meets with a patient during a visit, he uses the Snapshot feature to identify any potential issues that he needs to be aware of and discuss with his patient. With just a few clicks, Jesse can view their care team and any healthcare encounters they had in the past year, providing him with a quick orientation before diving into their medical details to prepare for the appointment.
Beth Cares | Clinical Care Manager
As a clinical care manager for a surgical office, having access to existing Advance Health Care Directive records within Connie allows me to initiate informed conversation with my patients about their advance care wishes.
Nancy Williams | Primary Care
Nancy is a care coordinator for a large primary care clinic. In an effort to provide the best possible service to her patients, Nancy uses Connie's Provider Directory when referring patients to a specialist so that she can quickly determine if the receiving provider accepts Medicare. Incorporating this simple and effective step into Nancy's workflow streamlines the process for the patient and helps to promote positive patient outcomes.
